Registration Instructions
Hello my name is Brenda Howell and I am the registration contact person for
the Family Matter's Retreat!  Feel free to e-mail me at
info@familymattersretreat.com and I will do my best to help you in any way
I can.  If you would rather call on the phone, our phone number is (423)
476-8941 and our fax is (423) 476-3349.  We want to make the registration
experience as easy and painless as possible.  With the help of the
instructions below we hope you will be able to assist your congregation in the
details of the retreat.  Anytime you e-mail me please be sure to put "Winter
Retreat 200
9" in the subject line.  
General Registration Information
1.       It is important that we have the name, phone number, and e-mail address of one
contact person in your congregation.
 This person or a predetermined representative will be
responsible for getting us the registration information as well as picking up the registration materials at
the retreat.  
Only this contact person will be allowed to obtain the registration materials.

2.         Please be aware that we must have T-Shirt sizes for your group in by January 29, 2009, or we
will not be able to guarantee them.  We usually order extra, however, we can not guarantee shirts for
those who do not turn in a size by
February 1.  Be sure to get your number of T-Shirts and sizes
to us by the deadline (
January 29, 2009).

3.        Make as many copies of the "Registration Form" as you need.  The "Registration Form"
should be used to sign up those of your congregation who wish to attend.  We hope that everyone will
take advantage of the "Registration" form on this website to submit your information, however, you can
still send it to us through the mail.

            a.          Each congregation is responsible for collecting each participant's money.
                         We ask that you send one check for the full amount.
                         The check should be made out to Central Church of Christ/Family
                         Matters, 200 25th Street NW, Cleveland, TN  37311

            b.         The Registration fee of $20.00 per participant is to be post-
                         marked by February 9, 2009.   Late Registration (after the 7th) is
                         $25.00 per person.        

            c.          There are no refunds for  cancellations!  You may transfer the fee from
                         a canceled participant to an added participant, but you may not assign them
                         to a room (See highlighted and underlined portion of Item 4 below.)

4.       Each congregation is responsible for making their own room accomodations.  You may
go to the hotel page of this website to find a link to hotels, cabins and lodges.

Please note:  Children five and under will not receive packets or t-shirts, however, we will need them
listed in your registration sheet for class preparation.
